What Is Dry Needling Therapy?

Dry Needling is a modern, evidence-based intervention used to treat muscular pain and movement impairments. It involves inserting a very thin, "dry" filament needle (one without medication or injection) through the skin into a myofascial trigger point—commonly known as a muscle "knot."

At Ang Physiotherapy, we use dry needling to target deep muscles that are difficult to reach with manual massage. When a muscle is overused or injured, it stays in a constant "contracted" state, which cuts off its own blood supply and causes pain. Dry needling "resets" the muscle, forcing it to relax and allowing fresh blood to flow back in.
 

How Does It Work?

Dry needling triggers several biological responses to stop the cycle of chronic pain:

  1. The Local Twitch Response (LTR): When the needle hits a trigger point, the muscle performs a quick, involuntary contraction. This "twitch" is the muscle's way of releasing the pent-up tension and chemical waste (like lactic acid) trapped inside.

  2. Neurological Reset: The needle stimulates specific nerve fibers that send a signal to the brain to release natural painkillers (endorphins) and "gate" the pain signals coming from that area.

  3. Increased Micro-Circulation: The tiny "micro-trauma" caused by the needle tells the body to send a rush of oxygen-rich blood to the area, which jumpstarts the healing process.

  4. Chemical Normalization: It helps balance the pH levels within the muscle tissue, reducing the acidity that causes the burning sensation associated with knots.
     

What Are The Treatment Techniques?

Our specialists use different techniques depending on the depth and sensitivity of the pain:

  • Superficial Dry Needling: The needle is inserted just below the skin over the painful area. This is used for sensitive areas or to reduce overall nerve pain.

  • Deep Tissue Needling: The needle goes directly into the thick of the muscle to find the core of the trigger point.

  • Pistoning Technique: The therapist moves the needle up and down gently to elicit multiple "twitches," ensuring the entire knot is released.

  • Needling with Electrical Stimulation: Attaching a portable TENS unit to the needles to provide rhythmic pulses, which is excellent for chronic stiffness and muscle "waking."

How Does The Patient Feel?

  • The Experience: The needles are much thinner than those used for injections; most people don't even feel them enter the skin.

  • The Sensation: When the "twitch" happens, you might feel a brief, dull ache or a "cramp-like" sensation. This lasts only a second and is a sign that the treatment is working.

  • Afterward: The area may feel slightly "heavy" or like you've had a hard workout for 12–24 hours. Once this passes, the original sharp pain is usually significantly reduced.
     

The Advanced Plan at Ang Physiotherapy

We don't use dry needling in isolation; it’s part of a "Total Reset" plan:

  1. Advance Combination Therapy: We often use Ultrasound before needling to warm the tissue, followed by needling to release the deep knots.

  2. Laser Therapy: Applied after the needles are removed to settle the tissue and reduce any post-treatment soreness.

  3. Kinesiology Taping: To support the newly relaxed muscle and maintain the "open" circulation created by the needles.

  4. Corrective Exercise: Once the knot is released, we teach you specific stretches to prevent the muscle from "locking up" again.
     

Contraindications & Precautions

Dry needling is very safe, but we take extra care in the following cases:

  • Blood Thinners: Patients on certain medications must be cleared for deep needling.

  • Needle Phobia: We offer alternative techniques like Shockwave Therapy if you are uncomfortable with needles.

  • Pregnancy: We avoid needling certain areas that could trigger unwanted systemic responses.

  • Active Infections: We never needle through broken skin or infected areas.

  • Compromised Immune System: Patients with certain conditions must be screened for safety.
